Dow Corning Trust: Payment options and medical criteria

DISEASE OPTION 1: PAYMENT AMOUNTS

Any approved disease in Disease Option 1 with a Severity or Disability Level of A, B, C, or D To be eligible for this, you must have proof that you have or had one or more Dow Corning breast implants and have not had a Bristol, Baxter or 3M silicone gel breast implant **

** If you have acceptable proof that you have or had a Bristol, Baxter, or 3M silicone gel breast implant, the Total Payment Amount will be reduced by 50%.
Note: If you received a Disease Option 1 Payment, you cannot later receive payment for one (1) of the diseases or conditions in Disease Option 2.

Locate your approved disease or condition in Disease Option 2 below and the Severity Level of that disease or condition. You must have proof that you have or had a Dow Corning breast implant and have not had a Bristol, Baxter or 3M silicone gel breast implant.**

Expedited Release Checks Important Please Read

The Settlement Facility Dow Corning Trust has recently decided to send Expedited Release Payments ($2,000) to all claimants whose cases are idle.

The Dow Settlement criteria states that if you accept this payment you can NEVER apply for a disease claim in the future, even if you become sick.

If you receive one of these checks in the mail, please do not cash it until you speak with an attorney to discuss your potential claim.

Again, if you cash this check you have effectively settled your claim and can never get any additional money.
